How to Schedule an Appointment

We see patients of all ages. We welcome self-referred patients as well as doctor referrals. Routine appointments are available Monday through Friday. Same-day appointments are available for patients requiring emergency care. Please call our clinic to set up your appointment.

What to Bring

• Current photo identification
• Insurance card (see accepted insurance carriers)
• List of the medications your are currently taking
• Patient forms

When You Arrive

We ask that patients arrive 15 minutes prior to their scheduled appointment time. Please sign in at our front desk. This allows us to review all necessary forms, verify your insurance and collect your co-pay. If you must cancel an appointment, we ask that you please notify us 24 hours in advance.

Before You Leave the Office

Please stop at our front desk before you leave. Follow-up appointments can be scheduled at that time. Depending on your insurance, payment for your visit may also be collected. Our billing department is here to work with you regarding any payment concerns. We accept cash, checks and most credit cards.

Emergency Care

For patients requiring same day appointments or urgent issues, please call the main number and ask to speak with a nurse.
